W-9 / Tax Information

Any direct payments from QVS which are not reimbursement for expenses (typically the monthly personal stipend, plus any loan support received) are considered taxable income. Though this is a small amount of money, it still meets the threshold for reporting to the IRS on a 1099-MISC form. Please fill out the W9 form so we may issue tax documents to you at the end of each calendar year. 1099s for the current calendar year will be mailed to your QVS house address, and 1099s for the second half of your QVS year (next calendar year) will be mailed to the address indicated on your W9, so we encourage you to use a permanent address on the form (we will also check in with you at the end of the year about the best address to use).
